How to Switch Your Home Care Package Provider in 4 Easy Steps

With a consumer-directed care scheme, you have the right to select your care provider and the flexibility to switch providers at any time. You don’t need a specific reason to change; the decision is entirely yours! Switching home care providers might seem daunting, but the process is straightforward and stress-free. Follow these four simple steps to make the transition smooth:

1. Review Your Current Home Care Package Contract

Check your current Home Care Package Agreement for any required notice periods with your current provider. Following changes made in September 2021, most funds are held by Services Australia in your Home Care Account. Once your old care provider finalises your account, the funds will be available in your Services Australia account for your new care provider within 70 days of your final day of service.

2. Research New Home Care Package Providers

Look into potential new providers who offer the services and care you need. Make sure you understand the difference between self-managed and fully managed home care plans. There are some helpful tools you can use to choose a home care package provider. My Aged Care’s Find a Provider tool is user-friendly and will show you all the providers in your area. 3. Notify Your Current Home Care Provider and Contact My Aged Care

Inform your current provider of your decision to switch by giving them the required notice as per your service agreement. This ensures a smooth transition and prevents any service interruptions. Notify My Aged Care of your decision so they can update your records. Re-activate your referral code and provide it to your new home care package provider. 4. Start Receiving Care from Your New Home Care Package Provider

Begin receiving care under your new home care package provider on the start date you’ve agreed on. This start date must be after the end date agreed with your previous provider to avoid overlap. Regular communication with your new provider will help ensure a smooth transition and continuous care.
